How to Refer

Complete all sections of Direct Access Echocardiography request form:

Direct Access EchocardiographyRequest Form NEW (2)

Attach ECG (from within 3 months)

Request via Choose and book to RMS

Results

The results will be emailed to the referring GP within 10 working days. This will include the technical report and advice from a consultant cardiologist. Should copies need to be sent elsewhere or results required in a shorter timescale please contact our administrator Jemma Cassidy on jemma.cassidy@nhs.net.

Should you require copies of historic letters or reports  please do not hesitate to contact us. We will need the patients name and NHS number to complete the request.

Changing An Appointment

Should a patient wish to change their appointment date, time or location please contact Referral Management on 01872 226700.
